New center offers help for dogs consumed by fear

(AP) ? People want their dog to be a friend, not afraid.

But sometimes, fear grips dogs so tightly they shake, cower, bite, growl or pee. It can be constant, painful and hard to overcome. Such dread can consume a dog when it's freed from a cage at a puppy mill or hoarder's home because that's the only life the dog has ever known.

Until now, it was up to animal shelters to ease the fears, knowing if they didn't, euthanasia was the likely alternative. But this week, the American Society for the Prevention of Cruelty to Animals opens its Behavioral Rehabilitation Center at St. Hubert's Animal Welfare Center in Madison, N.J.

It's a two-year research project being financed by the ASPCA.

For now, dogs seized from puppy mills and hoarders will be the primary patients, said Kristen Collins, ASPCA's director of anti-cruelty behavior rehabilitation and director of the center. It will also include some dogs that have been confined for long stretches as evidence in court cases.

Dogs will come from shelters across the country as well as from seizures involving the ASPCA.

It's groundbreaking and exciting, Collins said. "It's the first ever facility that's dedicated strictly to providing rehabilitation for dogs that are victims of animal cruelty."

The research will also provide some numbers, Collins said. No one knows how many shy dogs are being placed in homes now. And little is known about how they fare after placement, so center staff will spend a lot of time following up on animals.

There are 27 kennels, an office, real life rooms, treatment rooms and common areas at the center.

The average stay for most dogs will be six to eight weeks, "but we don't have a hard and fast rule. All dogs are individuals. We will be flexible," Collins said.

A team of 10 people, including two behavior experts from St. Hubert's, will staff the center. Volunteers and daily caretakers will feed the dogs and clean kennels.

Graduating dogs will return to a shelter for placement and ASPCA shelter partners will continue working with the dogs if needed, Collins said.

St. Hubert's is a longtime disaster partner of the ASPCA and jumped at the chance to be involved, said President and CEO Heather Cammisa.

Fear and anxiety are major factors that can hinder a dog's quality of life, she said.

"If they are hiding in the back of the cage and they are fearful, No. 1, they don't have a good quality of life and, No. 2, they are not going to be selected for adoption and when they go home, they are not really prepared to be the family pet that adopters seek, so this is just a win-all-around," she said.

The ASPCA spent over half a million dollars on the building, Cammisa said, and will pay all other expenses, including vaccinations, spaying or neutering, treatments and other care.

Weather permitting, the first few dogs will arrive in the next day or two from the Pacific Northwest, Collins said.

They will be the last of 213 Alaskan malamutes seized from a Montana breeder who was convicted in December 2012 of 91 counts of animal cruelty. After being starved and living in filth at the breeding facility, the dogs then had to be kept in kennels as evidence for 16 months while the trial played out.

Malamutes are 75-pound dogs. "Eighteen of the dogs were pregnant. One pregnant dog only weighed 48 pounds and had eight pups. Only one survived," said Bob Sutherland of Anchorage, president of the Alaska Malamute Assistance League.

The dogs were released to a humane society in Helena, Mont., where they were spayed and neutered, and another group helped place the animals.

While some dogs are in malamute rescues waiting for the right owner, many have found forever homes. Sutherland and his wife, Nicole McCullough, adopted one.

When the dogs were in evidence custody, Sutherland would visit to help out once a month. Cinder, a 6-year-old female, became his special project.

She is missing the tip of her ear, has broken teeth and a broken toe, injuries Sutherland said were caused when what little food was given to the dogs was thrown over a fence, causing food fights. Many of the dogs are even missing their tongues, he said.

Cinder has come a long way. "We took a shy dog, and she's all grins and giggles now. If you work with these dogs, they rise and shine. That's why this ASPCA facility is so valuable to us. We were super excited to get these dogs in there to go through a training regimen. It saves us a lot of heartbreak about what we do with these dogs," Sutherland said.

There will be those dogs that cannot overcome the fear, Collins said. But behaviorists will do everything possible and consider euthanasia as a last resort only if the dogs are suffering from an extremely poor quality of life or if they pose a significant threat to the public, she said.

The center will only be able to handle about 400 dogs during the project's two scheduled years, so it won't take an immediate burden off shelters, Collins said, but if researchers can come up with new ways to ease fear, anxiety and shyness in abused dogs, it could have a widespread impact.

And success could mean another phase in the study, to include fighting dogs, or even cats, Collins said.

Interoperability Unbound: CommonWell Health Alliance Unveiled at ...

Today, six EHR companies announced their formation of the CommonWell Health Alliance?to promote seamless interoperability of electronic health records.

Top health care information technology (HIT) companies Cerner, McKesson, Allscripts, athenahealth, Greenway Medical Technologies? and RelayHealth announced today the launch of the CommonWell Health Alliance?, planned to be an independent not-for-profit organization that will support universal, trusted access to health care data through seamless interoperability. This historic effort is aimed at improving the quality of care delivery while working to lower costs for care providers, patients and the industry as a whole.

The Alliance intends to be a collaborative effort of suppliers who are focused on achieving data liquidity between systems, in compliance with patient authorizations. The Alliance will define, promote and certify a national infrastructure with common platforms and policies. It also will ensure that HIT products displaying the Alliance seal are certified to work on the national infrastructure.

...

Elements of the Alliance?s national infrastructure will be tested in a local pilot within the next year. Early components will include the following core services:

  • Cross-entity patient linking and matching services:?Help developers and providers link and match patients as they transition through care facilities, regardless of the underlying software system
  • Patient consent and data access management:?Foster HIPAA-compliant and simple patient-centered management of data sharing consents and authorizations
  • Patient record locator and directed query services:?Help providers deliver a history of recent patient care encounters, and, with appropriate authorization, patient data across multiple providers and episodes of care

As I commented over there:

Promoting greater interoperability of electronic health records is one of the goals of the meaningful use regulations. Unfortunately, because of a combination of factors the standards for interoperability leave something to be desired, both in the stage 2 regulations and in the draft stage 3 standards. One factor is the tight deadline under the law for achieving meaningful use, and the other is the determination of the federal government to get as many folks qualified for meaningful use incentive payments as possible ? both factors tend to reduce the strength of the criteria against which EHR systems are judged.

The Direct project and other initiatives can already point to some success in this arena.

More meaningful success in this arena, however, has been left up to individual EHR vendors. The development of this new alliance is not, strictly speaking, in response to a regulatory requirement or deadline. So the question arises: Why now? The answer is that the EHR vendors must see a competitive advantage in banding together in this way, and this changed view of the world may be credited, in part, to the demands of patients (and clinicians) for interoperable EHRs and all the benefits that are supposed to flow from their ubiquitous appearance.

Let?s not forget that one revenue stream for cloud-based EHR vendors may be the licensing the use of de-identified patient data (license fees to be shared with, or perhaps retained entirely by, providers). And let?s not forget that one source of the growth in our collective knowledge, and improved evidence-based medicine, will be the licensed use of such data by third parties who aggregate and analyze data extracted from EHRs.

Bottom line: it appears to me that the EHR vendors are acting based on a multiplicity of motives; it is gratifying to know that at least one of these motives is related to market demands generated by consumers.

This is a promising step, but in some respects it appears to be a defensive maneuver directed at a dominant "big iron" EHR vendor notably absent from the group (Epic). There will probably still be work to be done by interoperability standards committees beyond the Alliance.

Interestingly, one of these vendors (athenahealth) also announced at HIMSS an HIT Code of Conduct, calliing on all vendors to meet its provisions as a way to respond to National Coordinator for Health IT Farzad Mostashari's recent challenge to the industry to go beyond what is required by regulation in building the health IT of the future. Specifically, the presser identifies the following core elements:?

  • Empower Data Portability and Provider Choice
  • Build a True Nationwide Information Backbone
  • Protect Patients
  • Prevent Fraud
  • Drive Meaningful Use

Other vendors at HIMSS announced the formation of the imPatient Movement, advocating for data portability.

Again, these developments are all driven by the evolution in the national conversation about health IT and a growing recognition of the need for patient-centric solutions.

'Selfish' gene may undermine genome police

Mar. 5, 2013 ? For a bunch of inanimate chemical compounds, the nucleic and amino acids caught up in the infamous "selfish" segregation distorter (SD) saga have put on quite a soap opera for biologists since the phenomenon was discovered in fruit flies 50 years ago. A new study, a highlight in the March issue of the journal Genetics, provides the latest plot twist.

In TV listings the series would be described this way: "A gene exploits a rival gene's excesses, sabotaging any sperm that bear a rival's chromosome." The listing is not an exaggeration except for ascribing malicious intent to strings of biochemicals. When male flies make their sperm, the SD gene (call it "A") manages to rig meiosis -- the specialized cell division that makes sex cells -- so that maturing sperm that bear chromosomes with the susceptible allele (call that one "a") end up defective and discarded. They never even leave the testes.

It is murder of a sort. Similar selfish systems occur in mammals, including humans.

In the Genetics study conducted at Brown University, scientists uncover new clues about how the SD gene might be gaming the system against "a." It's a plot so fiendish, only an aggregation of genetic bases could evolve it. It also deepens biologists' understanding of an instance in which life violates a fundamental balance predicted by the father of genetics, Gregor Mendel.

"Mendel's first law is that different alleles of a gene will segregate," said Robert Reeenan, professor of biology and the study's senior author. "If we have two alleles -- big A and little a -- then Mendel says 50 percent of the sperm at random will get the big A and 50 percent of the sperm will get the little a. But some SD (A) alleles are so strong they pretty much kill off all the non-SD (a) chromosomes.

"This is a real cheater, a real stinker," Reenan said. "Most genes, like most people, are good upstanding citizens, but some genes want to hog all the resources, hog all the benefit."

The SD backstory

What makes the "a" allele susceptible to SD's subterfuge is the number of copies it harbors of a runaway snippet of genetic code called Responder. A few copies of Responder are no problem, but hundreds of copies make "a" susceptible. Some alleles have thousands of copies and only one in a thousand survives.

Genomes try to root out parasites like Responder by creating and dispatching proteins into the nucleus and the cytoplasm. These police proteins are armed with "police sketches" of the parasites in the form of small RNA transcripts.

The new plot twist

It struck Reenan and lead author Selena Gell that this policing system -- because it targets self-copiers like Responder -- might somehow have a role in the SD saga. They decided to find out by purposely perturbing the system.

In the experiments described in Genetics, Reenan and Gell show that engineered mutations in the police gene named Aubergine (others on the force in the experiments are called Piwi, Squash, and Zucchini) amplify SD chromosomes' success in eliminating Responder-laden sperm, compared to that of SD chromosomes without Aubergine's help. The results show that this police system suppresses Responder, and therefore SD. It also means that if SD somehow can upset the policing system, it can have a field day.

"We're the first to have experimentally shown that mutations in the system can modify the degree of distortion," Reenan said. "We used homologous recombination to knock in a mutation specifically on the SD chromosome to compromise Aubergine, and that's exactly what we saw: the chromosome became more selfish."

Reenan and Gell did not go so far as to determine whether known SD-promoting genes called Enhancer of SD, Stabilizer of SD, and Modifier of SD act by interfering with Aubergine or its buddies on the force, but Reenan said that is among the next things his group will look into.

In the meantime, he reflects, it may not be entirely fair for biologists to label SD as "selfish" and not Responder as well. As an out-of-control self-repeater in the genome, Responder is surely no prize, and SD performs something of a service by taking it out when it can.

The whole story is really a clash of the selfish. "Humans, flies, all of us have been attacked for millennia by selfish genetic elements that want to make as many copies as possible," Reenan said.

Sometimes, as in SD flies, there are no apparent ill effects, but when the selfish genes come in the form of viruses or other kinds of transposons, there can be trouble. So investigating the tactics of selfish genes is not merely the stuff of biological soap operas.

Gell, who was supported by a National Science Foundation Predoctoral Fellowship during the research, is now a postdoctoral scholar at Harvard University.

Sony Ericsson Windows Phone prototype hits eBay, reminds us sliders existed

eBay is as close as it comes to a genuine Aladdin's cave, and we've seen plenty of ancient rarities, prototypes, sci-fi weaponry, and the odd killer robot go under its gavel. One of the latest artifacts of interest comes from eBay's Netherlands site, which is hosting an auction for a Windows Phone prototype slider known to her friends as Julie (or Jolie, depending on where you look in the listing) from the now defunct Sony Ericsson partnership. The phone that never was from the company that is no longer is allegedly one of only seven units made, and is touted as having an 8-megapixel shooter and 16 gigs of storage. Some digging through the XDA Developers' forum suggests the handset's old Windows Phone 7 ROM is basically non-functional, so don't expect to plug in your SIM and stroll out the door with a useable device. If that doesn't put you off, however, there's no exorbitant entry price, and bids remain sensible, for now. Head to the listing below for more pictures and to get in on the action, but bear in mind the only shipping options are for Europe. Nothing a PM with an outrageous offer won't rectify, surely.

Update: The seller has been in contact to let us know that international shipping is now available, and while the WP7 ROM running on the handset is by no means a final build, there are no issues with voice calling, the camera or Bluetooth.
